Healthcare Utilisation Questionnaire

Dear Participant,
You are invited to participate in a research study. I am an MSc Health Psychology student conducting research for my dissertation and not associated with any medical service or provision. I am interested in finding out more about health care utilisation behaviours.

The responses you provide to the questionnaire will be confidential and you will remain completely anonymous. Only the responses to the questionnaire will be recorded. You can withdraw from the study at any time and a debrief sheet is available on request by contacting myself on 05017963 @ glam.ac.uk.

This study has received ethical approval from the University of Glamorgan's Research Ethics Committee.

The questionnaire should take no more than 20 minutes to complete and your participation is very much appreciated.
